4.5A ready-to-use meeting notes template plus a short standard operating procedure for running it, so every meeting ends with clear decisions, owned action items, and a record anyone can scan in thirty seconds. Most meeting notes are a wall of paragraphs nobody rereads, and the action items live in someone's memory until they evaporate. This template fixes that with a fixed structure: a one-line purpose, the decisions made, action items with an owner and a due date, open questions, and a parking lot for off-topic tangents. The SOP explains what to capture live versus after, how to assign owners without awkwardness, and how to distribute notes so they actually get read. It is written for anyone who runs or attends recurring meetings: team leads, project managers, founders, committees, and remote teams. It works in any notes tool, a shared doc, or on paper. The outcome is meetings that produce decisions instead of vague 'good discussions', action items that have a single named owner and a date, and a searchable trail you can point back to when someone asks 'what did we decide?'. Adopt it for a recurring meeting and watch follow-through improve within two or three sessions.
Yes. The structure scales from a two-person check-in to a project steering meeting. For very short stand-ups you may only fill the action-items section, which is fine.
Assign one note-taker per meeting, named in advance. Rotating the role across a team works well. The worst outcome is everyone assuming someone else is capturing, so nobody does.
Every action item gets exactly one owner, stated as a fact, not a punishment. 'Owner: Sara, due Friday' is neutral logistics. Items with no owner do not get done, so the SOP refuses to record an action without one.
No catch. It is a complete, usable template offered free as a sample of the quality in the Productivity Templates collection.
